If pregnancy may be in periods
No, it can start at any time. You might have thought your period starts on the first of each month because periods are "monthly," but this just means that the average woman gets their period every 28 days. Once you get your first period, it will take about a year for it to regulate. Once it does, you will get a period around the same time each month, or you will have about the same number of days between each one.
It can be possible that a woman can have a period twice in one month. I would talk to a doctor.

It is rather unusal. Since you don't give much information I wonder if you aren't just having the same period. Sometimes a period will stop and then start again and that seems like two periods, but it is only the same one. Having sex, getting sick, and being under stress will do this to a period.
Yes, they do. Since the period is a natural cycle that a woman has, she indeed will still get her period even if she is in a coma. However, it might be irregular and the flow will vary from month to month.
No, not every woman has a menstrual cycle. Most healthy adult cis-women will menstruate, however women who have reached menopause will no longer have a menstrual cycle, nor will women who have had a hysterectomy - basically women who are no longer fertile. Women who use hormonal birth control don't have a menstrual cycle, as the purpose of hormonal birth control is to suppress the menstrual cycle so they don't ovulate - the bleeding women get is a withdrawal bleed, it mimics menstruation but isn't the same as menstruation. Many women were born biologically male, thus regardless of changes to their gender or genital reassignment surgery to change their sex, they are not capable of ovulation and thus don't have a menstrual cycle - although interestingly hormones given to many transsexual women can create symptoms similar to that of the menstrual cycle.
The only thing that happens exactly one month befor a woman's period is her previous period.

This is quite common. You may have mutli-cystic ovaries, or other sitiuations (that are not a worry) could be going on, such as; you might have been pregnant and had a miscarriage (very common, most women miscarry 3-4 times before they actually get truly pregnant). You may also just have had spotting. If this continues, see a doctor and ask if going on the birth control pill might help as it often settles down bleeding and periods into single events. Yes, a woman can go many months or even years without her periods - for example if a woman is pregnant she can go 9 months plus additional months after childbirth while hormones regulate, or if a woman is ill her cycles can stop. The average menstrual cycle length is 28 days, about a month, so a month between periods is completely normal - if you go for more than three months without a period it's a good idea to talk to your doctor.
