Tyler the Great Warrior was made via the Make-a-Wish Foundation by Tyler Gressle. At the time, the fourteen year-old boy was diagnosed with an illness that required a removal of 25% of his liver. While he was hospitalized, the Make-a-Wish Foundation cooperated with the Yu-Gi-Oh! company and allowed him to design his own Yu-Gi-Oh! card. The artists at Konami perfected and finalized his drawing and created one copy of the card.
Currently, Tyler himself is in possession of it. Although there is a large market for the card from collectors, Tyler was quoted as saying that is is not for sale.

There really isn't a way to get yugioh cards published. If you have an idea for a yugioh card, I'm pretty sure you can try e-mailing or writing to Shonen-Jump or Konami. The only other card published by request from somebody other then the company themself was a child in the Make-a-Wish Foundation who wished for a Yu-Gi-Oh card named after him. Yu-Gi-Oh then produced one copy of Tyler the Great Warrior.
