It all depends on the tokens being used. They can be used to Tribute Summon unless said otherwise on the card. For example, the card "Scapegoat" says: "...The tokens [summoned by this card] cannot be used as a Tribute for a Tribute Summon."
Take note of this last condition (a Tribute for a Tribute Summon). That means the tokens can be tributed for a Special Summon for monsters such as "Destiny Hero - Plasma".
Tokens that can be used for a Tribute Summon include: "Stray Lambs", "Statue of the Wicked", "Fiend's Sanctuary", and "Dandylion".
When using metamorphosis the LV of the Fusion monster should be the same as the LV of the monster you are going to tribute.
The only way you can do it is by using an external effect. Ancient Rules can special summon it, for example, Mausoleum of the Emperor can let you pay 2000LP to Normal Summon it without tribute. Phoenix Gearfried's 'second summon' to activate its Gemini effect, does not require tribute. But unless you've got a specific way around it, you still need to tribute two monsters to summon it initially from hand.
No, because it wasn't 'properly' special summoned. To be able to special summon a Ritual Monster from the graveyard, it has to be initially summoned by Ritual Summon. Ritual Foregone does not Ritual Summon.
What's in italics is answering your question. The rest is just other helpful information. Answer from yugioh-card.com: If Ultimate Offering is face-up, you can activate its effect multiple times in the same chain. When you activate Ultimate Offering's effect, you pay the 500 Life Points as a cost at activation, and then the Summon occurs at resolution of Ultimate Offering's effect in that chain. This means that it is possible to Summon a monster in the middle of a chain with Ultimate Offering's effect. If you are using Ultimate Offering's effect to perform a Tribute Summon, you Tribute the monster when Ultimate Offering's effect RESOLVES and you perform the Tribute Summon, NOT when you activate Ultimate Offering's effect and pay the 500 Life Points. If your opponent activates a Trap Card, you can chain the effect of your face-up Ultimate Offering to the Trap Card to Tribute Summon Jinzo (if you had a monster on the field to Tribute) and negate the Trap Card's effect. You cannot flip Ultimate Offering face-up AND use its effect in the same chain; you must have Ultimate Offering face-up on the field before the current chain began. If you activate Ultimate Offering's effect to Summon, and your opponent chains Mystical Space Typhoon to destroy Ultimate Offering, the Summon does not resolve because Ultimate Offering is a Continuous Trap Card and is no longer on the field. You can activate the effect of a face-up "Ultimate Offering" during your Main Phase even if you have not done a Normal Summon yet this turn.
Ancient gear golem is easiest summoned using geartown which allows you to summon ancient gear monsters with one less tribute, ergo, you only need one monster to tribute to summon ancient gear golem. Ancient gear golem cannot be special summoned which make things difficult. Cards such as spell gear and ultimate ancient gear golem can special summon ancient gear golem from the deck and hand, or graveyard, respectively, but they require much more cost.

It is so good that it is now banned (september 2009 banlist). It could be used to OTK by using Demise or Rescue cat: Demise OTK 1. Use advanced ritual art to special summon Demise by discarding 2 mecanical chasers 2. Activate Demise's ability to clear the field 3. Summon Black Salvo and Retrieve a mecanical chaser 4. Synchro summon Dark Strike Fighter with black salvo and mecanical chaser 5. Attack with Demise then DSF 6. Tribute both for game Rescue Cat OTK 1. Summon Summoner Monk. Discard a spell to summon another 2. Discard one more spell and summon a Rescue Cat 3. Tribute rescue cat to summon 2 X saber airbellums 4. Synchro summon 2 DSF with all your monsters 5. Attack with both then tribute both monsters and you win if the field is clear
No, you must tribute another monster using cannon soldiers effect.

Soul Exchange lists no such requirement. If you wanted to tribute summon, you don't need a monster on your side of the field for their monster to 'take the place of', you are simply using their monster to pay the tribute cost.
Yes you can but you have to tribute one of your monsters, as well as the one of your opponent's monsters that you tributed using Soul Exchange.
Yes you can, Soul Exchange simply offers a monster on your opponents side of the field as a tribute, as well as any others you may control.
Yes, if you tribute Set them from hand, or flip them face-down using the effect of another card.
No, Divine Wrath can only be used against effects that activate. Most of the time, when a monster has a way to special summon itself, this is an inherent special summon, just like a normal summon, it does not use the chain. Because it doesn't use the chain, you can't chain an effect negation card to it, though you can use summon negation cards like Solemn Judgment. If the special summon is an activated effect (like Dandylion's effect to special summon tokens, or Gorz's effect to summon himself) then these can be stopped using Divine Wrath. But Cyber Dragon's special summon cannot.
