The president of district 13 and the rebel, president Coin, was the one behind the explosion that killed her. The reason she did that was to make Katniss believe that Snow did it and therefore, to make Katniss' hatred for snow stronger, and so that she can have Katniss behind her. Because Coin believed that Katniss wouldn't be loyal to her and wanted to do everything, and anything she could do that would make Katniss be on her side.

When Katniss and her gang were trying to get to President Snow's mansion, there was Capitol children being taken from there parents to make the shield around Snow so that the rebels couldn't get into the Capitol and attack Snow and then Katniss saw Prim and should for her then the Parachutes floated down and everyone reached up to grab one because the thought they would help them but the turned out to be bombs created by Beetee and Gale It's Presidents Coins fault though because she saw that prim as there and still released the bombs and then the massive explosion came and blew up many rebels and Capitol people and kids Katniss was there but survived but Prim did not make it President Coin wanted it to happen so it would weaken Katniss to take over and become President and rule everyone but she winded up finding out and killed Coin and that's how Prim sadly past
Prim dies in Mockingjay, at the end of chapter 24 and at the beginning of 25. She dies by being one of the nurses that come to help the Capitol children after the first round of parachutes have gone off, and gets killed by the second round. The first actual words of Prim's death were:
My name is Katniss Everdeen. I am seventeen years old. My home is District 12. There is no District 12. I am the Mockingjay. I brought down the Capitol. President Snow hates me. He killed my sister. Now I will kill him. And the Hunger Games will be over...
Gale gets a fancy job in District 2 because he cannot stand the suspense of going back to his old home of District 12, and because Katniss is angry with him because she knows that it was he and Beetee's bomb that killed Prim.
Oh, dude, Prim dies in Chapter 27 of The Hunger Games. It's like a real bummer, you know? But hey, at least she got a whole chapter dedicated to her, right? Silver linings and all that.
