The president of district 13 and the rebel, president Coin, was the one behind the explosion that killed her. The reason she did that was to make Katniss believe that Snow did it and therefore, to make Katniss' hatred for snow stronger, and so that she can have Katniss behind her. Because Coin believed that Katniss wouldn't be loyal to her and wanted to do everything, and anything she could do that would make Katniss be on her side.

Prim died in The Hunger Games: Mockingjay the book. Prim died because President Coin (the one who killed her) thought if Prim was dead, Katniss would be broken on the inside. But Coin's plan back-fired. Katniss thought Snow killed her so, it led to more hatred to Snow.
It means dear to her, and very significant as well. This theory proves its point because in the first book, The Hunger Games, or the movie, when Prim is chosen as tribute for the Hunger Games, Katniss volunteers because she doesn't want Prim to die. Without much chance of actually surviving, she goes out there anyways, in place of her sister.
