At the end of the Hunger Games themselves (near the end of the book), the rules change so that there can be only one victor. So, since Peeta and Katniss have to be "the two star-crossed lovers from District 12", they both deside to kill themselves instead of killing either one. As soon as they ALMOST ingest the explosive berries that killed Foxface, it is announced that there can be two victors. So they both live. Then, after they are sent back to the Capitol and are rich and famous, President Snow sees that "fake" suicide as a rebellion against him and the Capitol. Now, in the end/second book, Katniss has to prove to President Snow and the citizens of Panem that the almost-suicide was out of love, not rebellion. And since Peeta actually loves Katniss and she doesn't, it is complicated.
Prim died in The Hunger Games: Mockingjay the book. Prim died because President Coin (the one who killed her) thought if Prim was dead, Katniss would be broken on the inside. But Coin's plan back-fired. Katniss thought Snow killed her so, it led to more hatred to Snow.

Coin sent her there, wile knowing she was going to die. That's why Katniss shoot Coin, because she was sure Snow was going to die if she didn't shoot him. I personaly think Coin did it so Katniss would run to Prim, so that Katniss would die to, and why Coin would want Katniss dead, says the book right at the beginning of part 3.
At first, she thought that it was President Snow's army that killed Prim because of the Capitol's helicopter that she saw, but when she met President Snow, he told her that it wasn't him. He claimed that he saw Plutarch's hand as one that dropped the bombs on the innocent children and medics. I'm wasn't sure if Katniss knew who killed Prim, but when I read that she shot Coin, I got my answer. Katniss knew that Coin ordered for the children to be bombed, and Coin knew that Prim would be there to help the children. I think that this is what drove Katniss to kill Coin.

If you're talking about at the end of the whole series then.... Katniss and Peeta get married and have kids, Prim dies, President Snow is planned to be executed by Katniss shooting him but Katniss instead shoots President Coin (leader of the underground district 13), but President Snow laughs to death. That might be it but i dunno, its been a while since i read the books :P
Katniss kills President Coin instead of President Snow. When Katniss is in the hospital, Coin pays her a visit and tells her that the bomb that killed her sister Prim was an order from President Snow. She thinks back and remembers a conversation where President Snow says they should not lie to each other, and realizes that he did not order the bomb that killed Prim, but Coin did. The day she is to kill Snow, she instead raises her bow and kills Coin instead. There is a stampede of people who had gathered to watch Snow be killed fleeing from the scene. Snow is seen choking on blood as Coin collapses. After the ordeal settled down, Snow is found dead where he once stood, he either died choking on his own blood or by being run over by people in the hurry to run from Coin's assassination.
