Usually people with piercings are considered Gothic or emo, or some other stereotype. It makes people look unprofessional if they have nose, tongue or belly button piercings. You don't. That's just because most doctors are against piercings. You can always deny a ridiculous request like this and continue surgery with your nipple piercings in.
If you intend on keeping the piercings, jewellery should be left in the piercings, there is no reason to remove the jewellery except to get rid of the piercings. Piercings can take a full year to become seasoned to the point where jewellery can be removed for extended periods.
Well the piercings will not close up completely after that period of time. It can take a year or longer for piercings over a year old to shrink and close.

This depends on how long the piercings had jewellery in them, the longer the jewellery has been left in the piercings the longer the piercings will take to shrink and eventually close, months to years it all depends on time.
If you want to be able to put the jewellery back in you need to change the barbell to rings to maintain the piercings if they are under a year old. Over a year old your chances of loosing the piercings due to closure of the piercings isn't as great due to the piercing being more seasoned and toughened up.
