Currently, there are not any maps or bits of information specifying where the other districts are located. All we know is that District 12 is somewhere in Appalachia and the Capitol is located in the Rocky Mountains.
The whole of Panem reaches all across North America. Technically, it could reach up to Canada or down into Mexico. For example, District 11 is located in the south and is dedicated to agriculture, which could point to Texas or Mexico.
Most fan-made theories say that District 6 (Transportation) is located in the midwest region of the United States - most likely because of the automobile industry in Michigan and due to the fact that a lot of factories transport their goods out of the midwest. This is only a very popular theory among fans, and we will only know the exact answer when the actual map is released on The Hunger Games Adventures (Facebook Game).

Well, the adventure is about the country of Panem, and there was 13 districts in Panem. In District 12, a poor district, there lives Katniss Everdeen, and Peeta Mellark. In a time of history for Panem, ( called the Dark Days ) the Capitol destroyed District 13. The Capitol says that it will never happen again, and so came The Hunger Games, where each district offers up one boy and one girl to compete. Katniss and Peeta are chosen for District 12. The Hunger Games is a fight to the death in an area made by the gamemakers. there are 24 tributes ages raging 12-18. The story tells how Katniss tries to survive and win the Hunger Games. The official map of Panem, as shown in The Hunger Games Adventures shows that District Four is on the western coast of Panem (North America). Between D4 and the Capitol is District 1. It takes less than a day to get from the east coast (district 12) to the Capitol, so I would estimate it would take only a couple of hours, if not less, as the trains travel at highs of 250mph.
