The whole of Panem reaches all across North America. Technically, it could reach up to Canada or down into Mexico. For example, District 11 is located in the south and is dedicated to agriculture, which could point to Texas or Mexico.
Most fan-made theories say that District 6 (Transportation) is located in the midwest region of the United States - most likely because of the automobile industry in Michigan and due to the fact that a lot of factories transport their goods out of the midwest. This is only a very popular theory among fans, and we will only know the exact answer when the actual map is released on The Hunger Games Adventures (Facebook Game).

It takes less than a day to get from the east coast (district 12) to the Capitol, so I would estimate it would take only a couple of hours, if not less, as the trains travel at highs of 250mph.
