To get the Black Belt item in Pokémon Ruby you need to beat a trainer called Black Belt Nob five times on Route 115. To get rematches with him you need the Trainer's Eyes function of the PokeNav, which will only work after you've gotten 5 badges. If you walk into and out of the Route enough times, he should eventually call your character to ask for a rematch. After beating him five times he should give you the Black Belt item.
