After time the skull will disappear if you don't attack anymore people during that time. When you are skulled and die you will lose all items unless you're using prayer to keep one extra. For this reason you must be careful when skulled, but it might be a risk worth taking depending on the situation (it is much less risky if you have teleport runes or are near a safe area).
