Name tags can only be put on animals, including hostile and neutral mobs as well as friendly ones, but not anything else! (like naming a door) To change the name from name tag to your own one go to an anvil and rename it. Then go to your animal and right click it, the name should pop up on top. If it's a horse you might start riding the horse instead of naming it, if so just right click its neck when you're on it and it should come up.
Also, as a note when you move away from the named mob if your mouse is not on the animal the name will not show up. The mob still has that name, it's just that you've moved out of sight. Move in again to reveal its name once more.
You can always rename the animal by renaming the name tag on the anvil.
