This is a rather complicated process. First, go to the nether, find a fortress, kills tons of wither skeletons until you get three of their heads. Next, grab four soul sand. Place two soul sand on top of each other. Next, place the remaining two soul sand blocks on the top block on each side. Then, place the three heads on the three blocks forming a "T." Then, run. The wither is very powerful, so be prepared.

well when 1.4 comes out it will take 4 soul sand in a t shape and put 3 wither skeleton skulls on top (the skull is a rare drop from a type of skeleton in the nether) you must have it on easy, medium or hard for it to work.
