Actually, the so called "Big Boss" face camo in Metal Gear Solid 4 is actually Solidus' face after his skin had been removed. You can acquire it by earning the BIG BOSS emblem. It works well with the death camo.

When you save your game after completing the game, you will be able to restart the game by loading your save file. When you do that, the items you got from the last game will be in your inventory at the start of the game, things like: The bandanna (infinite ammo), or the stealth camo (invisibility) obtained depending on which ending you get.
