Ushio had her mothers disease, ever since she was little. Now when she was terribly sick, she really wanted to go outside and go to a certian place, but in order to get there they had to walk for a bit. Of course it has to be in the dead of winter when it is snowing furiously; and she can not stand anymore. inevitabley she dies because of not being able to survive. (that sentence wsa obvious :p)

ohkay so Ushio doesnt actually die. IN the series she dies, but in real life she does not. As soon as Ushio was born, Tomoya ahd a dream/vision of some sorts, that Nagisa did not make it, and that inside the dream was how life would have been without her (all of what he felt it would be like). So at the very end you see that Ushio is still just a baby, and that Nagisa is still alive, Tomoya is happy that neither happend.
