Quite simply, the pilots were negligent in their preflight checks and did not mannually check the tanks for adiquate fuel levels. Several weeks before, the pilots had noticed poor combustion on the right engine. To correct this, they set it to an auto-rich mixture which consumed much more fuel then they anticipated.
