Harry Potter leaves from platform 9 and 3/4s. He and his Hogwart's companions get to this by running through a brick wall between platform 9 and 10.
In the second book, the Chamber of Secrets, Ron Weasley and Harry run at the wall, only to find that it has been sealed off. Their excuse to the muggle station officer who comes to them looking very worried is ' sorry, couldn't control the trolly.'

Platform nine and three quarters (9 3/4's) from King's Cross Station.
This platform is of course fictional however King's Cross have made a barrier with a trolley halfway through the wall and a banner over the top which says Platform 9 and 3/4's for tourists and fans to visit.
The Hogwarts Express always leaves from platform 93/4 at King's Cross station, London at 11:00 AM.
You have to push your trolley straight at the barrier between platforms 9 and 10...
He left at the end of Harry Potter and the Prisoner of Azkaban.
No, he does not. He, Ron Weasley, and Hermione Granger leave Hogwarts after their 6th year in order to hunt down Voldemort's Horcruxes. However, they do return for the Battle of Hogwarts, in which they fight against the Death Eaters alongside other Hogwarts students.

Yes. Hogwarts was evacuated and every student under the age of seventeen was supposed to leave along with anybody who didn't wish to fight. It would seem the majority of people over seventeen stayed and that many under that age sneaked back in. However it is hard to tell exactly how many students at Hogwarts died. Colin Creevy is the only confirmed person who was still in attendance at Hogwarts. Harry said there was fifty others who he didn't recognise so it's highly possible that several of them were Hogwarts students but unlikely that any of them were in their final year (seventeen or eighteen therefore allowed to be there) as Harry would have known them even if not by name by face. It is also likely that there were more than the fifty Harry saw as bodies were still to be recovered. However none of the unnamed people will be people Harry knew.
The Hogwarts Express always leaves on the 1st September no matter what day of the week it is.
