Katniss dropped a tracker jacker nest on Glimmer (A tracker jacker nest is a muttation of the Capitol, also used in wars) but Katniss got stung three times before the tracker jackers stung Glimmer. Glimmer died of the venom and too many stings.

Cato had a long and painful death. When he came through the forest and running at the Cornucopia, Katniss attempted to shoot him with an arrow but hit his body protector. Then the dangerous muttation wolves came and Cato scaled the Cornucopia. Each one had the traits of a dead tribute. Such as Glimmer's mutt, it had blonde hair and her sea green eyes. Katniss pushed Cato off and into the pack who nibbled him to a chunk of meat. Katniss and Peeta returned to their cave expecting him to die but when the muttations left Katniss had to shoot her final arrow at Cato and plunged it into his head
Rue's last name is actually not revealed. Most of the characters who participate in the games with Katniss's last names are actually never revelaed. For example; Clove, Cato, Glimmer, Marvel, Cashmere, Gloss, Enobaria, Brutus, Cecelia, Woof, Thresh, Seeder, Chaff and many others.
If you are refering to the movie it was three arrows. In the book they do not give a specific amount of arrows, but it seems like five or more. The book only says 'pierced by many.'

A quiver is an archer's case for carrying his arrows. Typically, it was made of leather, and carried on the back over the shoulder. It was standard equipment for hunters and soldiers. There are many types of quivers, with the most common being the type which is hung from a belt; others are the bow quiver (attached to the bow), the back quiver (slung on the archer's back, like a knapsack), and the ground quiver (often just a length of stiff wire with a hoop on one end, the other end pushed into the ground). There are other types of quivers, but those listed in the preceding paragraph are the most common types.

A fireball brushes her calf, scalding her. She manages to put the fire out, but her hands and calf are seriously injured. Eventually the attack ends and Katniss walks until she finds a small pool. The water soothes the burns. She knows she should keep going, but the pain is too great when she takes her leg out of the pool, so she spends the day there recovering. In the afternoon, however, she hears footsteps, and without any alternative she finds a tree and climbs as quickly as possible.It’s the five Careers and Peeta. Katniss calls down to them, knowing they’re too heavy to climb to where she is. Katniss notices that a girl named Glimmer has the bow and arrows from the Cornucopia. A large and dangerous looking boy named Cato begins climbing up after Katniss, but he falls out of the tree. Glimmer goes up next, but she doesn’t get far before she must stop. She fires an arrow at Katniss, but she’s incompetent with the bow and her shot misses. The group decides to wait Katniss out. Unable to go anywhere, Katniss prepares her bed in the tree
