It's very symbolic actually. To have a love so amazing, it doesn't need to show that they need to kiss to prove it.
Ushio had her mothers disease, ever since she was little. Now when she was terribly sick, she really wanted to go outside and go to a certian place, but in order to get there they had to walk for a bit. Of course it has to be in the dead of winter when it is snowing furiously; and she can not stand anymore. inevitabley she dies because of not being able to survive. (that sentence wsa obvious :p) ***SPOILER ALERT*** seriously if you dont want to be spoiled dont read on ohkay so Ushio doesnt actually die. IN the series she dies, but in real life she does not. As soon as Ushio was born, Tomoya ahd a dream/vision of some sorts, that Nagisa did not make it, and that inside the dream was how life would have been without her (all of what he felt it would be like). So at the very end you see that Ushio is still just a baby, and that Nagisa is still alive, Tomoya is happy that neither happend.

It's very sad and yes nagisa dies after giving birth to ushio. It tears tamoya up and starts to work full time at his job, while blowing his money away on non-work days. Sometimes when Sanie (Nagisa's mom) would bring ushio over, tamoya would never even look at ushio. He starts to be like his father to her. It's sading that he wishes he never met Nagisa so she would still be alive and ushio was never born that way Nagisa would still ba alive. It's five years later and he never spent any time with ushio and wasnt like the father he wanted to be like with nagisa. Of course this is all in the AFTER STORY. NAGISA COMES BACK THOUGH.

Basically the lights in the other world saved them. Tomoya collected a bunch of them throughout his life from helping a bunch of people (such as Miyazawa and his own father). When Ushio died, he lost all hope in life but he wished that Nagisa hadn't died while giving birth to Ushio and that Ushio was still alive. When he died (yes, he died in the last scene while clutching Ushio), he made that wish and was whisked back in time to when he first met Nagisa and was given a choice to whether or not meet her and have all those things happen. He decided to meet Nagisa and he fast forwarded to when Nagisa had given birth to Ushio and she had survived. Also, just to clear things up about the robot and the girl in the other world (they played a big part in Nagisa's revival), the girl is Ushio and the robot was Tomoya. Both sides of Tomoya (the robot and the one in the real world) were collecting a bunch of those lights and together, they combined all their lights and made one wish each. Tomoya made the revival wish while the robot wished that he and the girl (ushio) could go to the other world. Since they collected so many lights, their wishes came true. The girl was born into the other world as Ushio while the robot was reborn as Tomoya.
