Well, in The Empire Strikes Back, in the battle of hoth, when Vader arrives at the ice planet, he says "The rebels are here. And I believe Skywalker is with them." He doesn't make the connection that he is Anakin Skywalker. He does think, however, that the one who took down the Death Star in A New Hope was special and he felt the force in him. After the battle of hoth, Vader spoke with the Emperor, and he tells Vader that Luke is the son of Anakin Skywalker. The Emperor said that he must be destroyed. I believe Vader was trying to save his son in saying "He could be a powerful ally." Then he offered Luke a rule as father and son, and Luke refused. Vader didn't find that Leia was his daughter until he felt the connection with her and Luke in the Emperor's throne room in ROTJ.
As far as I can tell, he knew that Luke was Anakin's son because of the relationship Anakin had with Padme. When Padme becomes pregnant, it is only a matter of time before Sidious figures it out. Also, when he finds out that Luke's last name is Skywalker, that was another sign. Not to mention the fact that he can use the "force" to sense the connection between them.
It could be argued both ways. But directly he did not. He convinced his father to come back to the good side. When he did Darth Vader killed the emperer. That caused Darth Vader to die because it was only through the emperer's power that he was being kept alive. He was injuring by lava in his fight against obi whan. So when he killed the emperer Darth Vader died. So technically nobody killed Darth Vader he just died. However it also could be argued Luke is to blame. If he had not convinced Darth Vader to come back to the light side of the force he would not have died.

in the end of star wars episode 6 nobody really kills Darth Vader/anakin skywalker but when he is throwing the emperor off the thing he gets shocked by his sith lightning and it keeps running trough his body because he is mostly made of metal so that's how he dies.In a way Darth Vader actually kills Anakin in Revenge of the Sith when he turns to the Dark Side of the Force, as hinted at by Obi-Wan Kenobi in Return of the Jedi.Anakin wasn't killed in the strictest definition. He redeemed himself when he saved his son Luke from being electrocuted to death by the Emperor, but doing so caused the lightning to compromise Vader's suit, which was pretty much a life support system. So Anakin's death was more of a sacrifice

Darth Vader, the Sith Lord that was Anakin Skywalker, became so because he surrendered himself to the Dark side based on his feelings of fear, doubt and hate. Thirty years later, when Lord Vader witnessed his son Luke throw down his light saber and refused to fight The Emperor, he also witnessed the true power of the Light side and saw in his own son, the Jedi that he was supposed to become - a powerful, mature, benevolent, fearless master of the Force, (much like his Master Obi-Wan Kenobi.) This demonstration and Luke's faith and continued insistence that Anakin could still be redeemed, is what caused Darth Vader to have respect, compassion, mercy and indeed love for his son in the moments that The Emperor was assaulting Luke with Force Lightning. These feelings were incompatible with and detrimental for the essence of a Sith Lord, but not for the essence of a Jedi. Luke knew this, and knew that the essence of his father still existed inside Darth Vader. Luke demonstrated this faith earlier when he turned his back to Vader during Luke's surrender, allowing Vader the opportunity to decisively end the conflict by killing Luke, who was unarmed while Vader held Luke's Light sabre. Luke's love and faith is was what destroyed Darth Vader and saved Anakin. Anakin in turn saved Luke, sacrificing himself and redeeming himself in the same selfless act.
