Scrooge does not really know how ill Tim is. The first time he has any contact with the Cratchit family is when he is taken there by the Ghost or Christmas Present. Scrooge sees how loving the family are and how ill Tim is. he asks the Ghost if Tim will get better? The Ghosts' only response is "I see an empty seat by the fire and an unused crutch on the wall before next Christmas." This upsets Scrooge and starts him re-evaluating how he can help.
