The uterus cannot tell when you are asleep or not, although circadian rhythms may effect flow and when laying down less blood flows down into your vagina. This is why people still use menstrual products while they sleep, and extra care often needs to be taken in case of leaking overnight. As a note tampons shouldn't be worn while sleeping as they can't be changed often enough.
