Depends on the country but usually means they are flavors derived from natural chemicals (natural flavor) or artificial man made chemicals (artificial flavors). A natural flavor MAY be healthier but it MAY also be unhealthier than artificial flavors. Depending on the country some natural flavors may be a mix of artificial and natural chemicals. In fact, an orange natural flavor will most likely be a mixture of chemicals tat have no relation whatsoever to an orange, or possibly not even a fruit.

As such it is a complete secret what the chemicals are and you are better avoiding natural and artificial chemicals altogether.

Natural flavors are derived from natural sources like fruits, vegetables, or herbs, while artificial flavors are chemically synthesized in a laboratory. Natural flavors are typically more costly and can provide a more authentic taste, while artificial flavors are often used to mimic specific flavors at a lower cost.

Is there a difference between natural and synthetic substances?

From a chemical standpoint, there's no difference in the substance itself. However, there may be differences in what impurities are present and their relative concentrations. One of the reasons artificial flavors tend to taste ... well, artificial ... is that they don't have the same blend of impurities as the natural substance does. Artificial vanilla flavoring is just vanillin, which is the main but not the only component in natural vanilla extract. This makes it more of a "one-note" flavor, without the subtleties present in real vanilla extract.
