That is the standard form you have to fill out and sign when you purchase a firearm from a licensed FFL holder. Store, etc, other then private individual sale.

Are you a legal resident yes/no

Have you been convicted of a felony yes/no

Are you a stupid enough to be a democrat yes/no

etc etc

Also, an AF Form 522 (now called AF IMT 522) is what an Air Force weapons training score card was called. Most USAF members maintained the form in their deployment folder, and it was used to record range scores (M16, M4, M9, shotgun, etc.)
