That is the standard form you have to fill out and sign when you purchase a firearm from a licensed FFL holder. Store, etc, other then private individual sale.

Have you been convicted of a felony yes/no

Are you a stupid enough to be a democrat yes/no

CORRECTION: The above is not the 522. That is DD Form 2760 - QUALIFICATION TO POSSESS FIREARMS OR AMMUNITION

The AF Form 522 is your weapons qualification history showing what you fired, when you fired, how much you fired, course of fire, and if you qualified in that weapon system.
