It's more often than not used where I'm from to refer to how well your clothes, hair or makeup suits you, but it can be used to compliment your look in general or be used to acknowledge and reward the effort you put in to look how you looked when he said it.
It's a rather feminine but perfectly acceptable compliment from a guy.
