Totally depends on how long of a "hit" or "puff" you take, and whether or not you inhale or not. Cigars are meant for "puffing" - not inhaling, generally, you breath in some smoke into your mouth, hold it for a couple seconds, then blow it out. You are not supposed to inhale the black and mild smoke into your lungs, though many amateurs - or those who've grown tired of "regular" puffing, and looking for more of a "buzz" will resort to inhaling, but the side effects (cancer, high blood pressure, etc.) and consequences are a lot greater if you inhale.

Only if you replace the tobacco with some sort of magic herb. Otherwise, it will probably make you very sick if you don't smoke much
