Lucky for you, I'm underweight so I'm just the person to answer your question. There are multiple things that can make a person underweight, for example, being a picky eater. If you rarely like the food you get and don't eat it, sooner or later you will be having horrible pains every day and you will not be able to eat anything, even though you need it. So to avoid that, just eat your favorite foods, until you're slightly overweight (still below average...) and then STOP before things get out of hand.
No. Anorexia is severe underweight. It has several causes, often psychological, but is not a cause itself.

Yes and no. If you are underweight and not healthy, then that's a problem. If you are healthy and have no issues according to your doctor, being underweight shouldn't be a problem. I am underweight by twenty pounds, and I have no issues besides ability to fight colds, but hey, don't we all?
