The fat in a cake can easily be replaced with healthy ingredients. For example, fat-free yogurt makes a great substitute for oil. Step 4 Use a sugar substitute. You can find store-bought sugar substitutes that taste just like sugar or you can use natural substitutes. Natural substitutes, such as honey or maple syrup, will add additional flavor to a healthy birthday cake. Step 5 Prepare a low-fat frosting. Even a healthy birthday cake can have a delicious frosting. You can use a fat-free whipped topping or you could also use fat-free cream cheese sweetened with maple syrup or honey
