Well that's a discussion you should have had with your piercer. See surface piercings are technically difficult piercings there are a couple ofdifferent ways to pierce them to get them to stay (not migrate). Depending on how yours was done, that will dictate what jewellery you can have in the piercing that will minimize the chances of migration. The punch and taper method has proven to be the most successful method holding the best over all results and the method I have chosen to use for surface work. So best bet is to have a chat with your piercer and ask what he/she would suggest.

2 weeks after the piercing was done, the swelling will be down by then and the barbell can be changed for a shorter one.
Your tongue will return to it's normal size and the barbell will begin to feel too long for your mouth. At this point it is time to go back to your piercer and have them change the barbell.
The tongue doesn't form Keloids, the bumps on your tongue are possibly due to mismatched metals in the barbell. This is called galvanic corrosion and will cause the tongue to react by forming lumps by the piercing. Purchase a good quality barbell from a professional body piercing studio and replace the barbell in your tongue. You should start to see a change in the lumps within 48~72 hours. Junk jewellery is everywhere, be sure to purchase quality jewellery from a reputable vendor like a body piercing studio.
Well you should step the barbell down two weeks post piercing after the swelling has gone down. So changing the jewellery is not an issue just make sure you are putting in good quality materials. Stay away from the junk jewellery.
Experienced professional piercers who know there stuff always do nipple piercing with barbells, being a straight piercing a barbell will allow for normal movement without the risk of snagging. They are far more comfortable during the initial healing stages then once the piercing has a good light heal (6~8 weeks) you can change the jewellery to a "D" ring with will offer the effect of a ring with the comfort of a barbell. Research your piercer and make sure you are dealing with an experienced piercer and not a novice piercer, you won't be disappointed.
Nipple piercings are straight piercings regardless of what jewellery is in the piercing when it was first done, the barbell allows the piercing to discharge and affords room for cleaning better than a ring does. Don't let anyone tell you that if the piercing is done with a curved needle the piercing is curved, that a line of BS. Once the piercing is healed you can change over to what is called a "D" ring this ring has a straight section that goes through the piercing and is by far more comfortable than a standard ring. It has the appearance of a ring but the straight segment allows the ring to be moved without twisting a curvature in the piercing like you would with a standard ring. Ultimately for day to day wear the barbell is going to be your jewelery of choice, for comfort and discretion.
