I gather the term refers to women acting in male roles. Not aware of any in opera. in Musical theatre, Mary Martin virtually created the role of Peter Pan- surely a male character, albeit juvenile. more recently a Sandy Denny or Dennis played the role of Pinocchio on Broadway or other live theatre. Both are juvenile parts played by adults. Shirley Temple played some male roles ( lLittle Lord Fauntleroy) some kids translated this as Falter-Joy! how is that for a pun. see how the pants fit.
The way it is played and held and displayed it appears to symbolize male genitals.

I have to start off by saying that there probably aren't many younger audiences that are watching Friends, since it is a TV14 show. However, I would say that for those who are part of a younger audience that did watch the episode, they probably felt that having a male nanny is unacceptable, as Ross did firmly believe that they could not have a male nanny for Emma because it was unnatural.

Billy Goat is the same as a Buck which are both male intact (with testicles) goats Nanny Goat is the same as Doe which are both female goats
A male goat is a billy-goat. A female goat is a nanny goat. There is no male nanny goat. Goats are often times, and most widely called "billy" for the male or "nanny" for the female. The more proper terminology is to refer to the fully functional male as a "buck", the castrated male as a "wether", the adult female as a "doe" and a newborn goat is referred to as a "kid". Terms such as "buckling" and "doeling" are often used as well to further describe the sex of a "kid".
