I had a very similar problem with my 1996 Nissan truck. New plugs, wires, distributor and rotor only partly solved the problem. Then I replaced the O2 sensor and the probelm disappeared completely. Had 130K miles on the truck when replaced and the O2 sensor was shot.

Start your investigation around the fuel filter, chances are your filter is dirty. Problem with 96-98 JGC is that the filter is built inside the fuel tank, so if it a big job to exchange filters. Try adding a fuel line cleaning solution to the vechile next time you fill up. ADDITION>>>> Look at your Tachometer when this happens, is it climbing? If it is look at your transmission, is it slipping? Do you have Quadra Trac? If so is your xfer case slipping? Do you hear a clunking when it jerks? If so is it front or rear end? there are MANY things that can cause a jerking under acceleration... is it hard acceleration? From a stop or during movement? More info is needed!
