In the absence of any chronic disease (such as Cerebral Palsy, or Asthma), most children (or adults) who gag when drinking milk likely have a lot of sinus drainage.

Children and adults who live in colder climates often have drier nasal passages from being in heated buildings. The dryness of the air can create an environment that makes it easier for children to develop ear and sinus problems.

Any child or adult who has ear problems (in any climate) often has eustachian tube dysfunction-- i.e. the ears don't drain fluids properly. This combines with sinus symptoms or infections to create more drainage in the throat.

Children or adults with Allergies may have more secretions also.

Children or adults with lots of sinus drainage may frequently cough or clear their throats. They may complain they feel a tickle in the throat or like something is "stuck" in the throat. The "stuck" feeling often indicates a glob of thick drainage from the back of the nasal passages / sinuses into the throat.

Milk is a thick viscous liquid, compared to water or juice. Milk hits the area of sinus drainage, and triggers the gag reflex because it's hard to swallow with the "gunk" sitting at the back of the mouth/throat.

Some people who gag on milk find it helpful to have a swallow of warm water in between swallows of milk; but other people find that a swallow of cold water in between milk breaks the gagging faster.

Other people like to drink something hot, like plain tea, before a meal that includes milk. The hot liquid helps break up secretions.

If this is a chronic problem or worsens in the winter, get a Dr evaluation to be sure the person doesn't have a bacterial sinus infection. A very low-dose decongestant often can dry the secretions. Some doctors prescribe a small dose of children's benedryl or other over the counter product. If you suspect allergies, get skin tests done. Allergy shots can help greatly.

Adding humidity to dry homes can help throughout the winter. Consult your doctor for the best plan of action.

Note: The same gagging effect can be caused by drinking any thick fluids, but milk is one of the top 3 offenders.
