Most shops won't do tattoos for people younger than 16. Some individual tattoo artists won't do tattoos for minors at all.
Either way, both you and your guardian need to be present--written consent is for skipping class, not for permanently altering your flesh.
I'm not sure what the laws are for other states, but in Minnesota both the guardian and the minor need to have photo IDs. Your school ID will suffice as long as it is a photo ID.
It's up to the studio as to what they will provide for you, but seeing you have a parent with you to sign off, then it's between you and the studio as to what they will do for you. Some piercings you just can't get regardless of the parent signing off or not.
Contact your local health inspection or health protection office, they will be able to tell you the age requirements for your area as well as provide a list of inspected facilities near you.
you want to be at least 18 just because when you are still young your body is still growing and stretching so your tattoo will stretch with it. plus when you are 18 you can go by yourself.
I think you have to be 18, most tattoo parlors will not do them until you are adult, scared of lawsuits,
in the United States it is 18 without parents permission, if you have consent you can get one under 18.

Your parents must be with you and you and your parents must both have ID and then any Virginia parlor will do it.

I know most tattoo parlors there will tattoo a 17 year old with parents PRESENT. Some will do 16 year olds with parents PRESENT. but written consent doesn't mean much when it can be forged.

The laws vary in the US from state to state. Some do not allow a minor to get a tattoo at all. Others allow a 16 year old to get one with permission. Some require the parents to be present when the tattoo is put on.
It might differ state to state. In New York state, you have to be 18 years old.
