I believe If we go by the last name of lets take as example the Royal Family of Fiore we know Fiore the kingdom is named after the royal family so by that and knowing Irene was the Queen of Dragnof we can asume Irenes full name could have been Irene E Dragnof meaning Erza would be named Erza E Dragnof. But that is all just an asumption. Until Mashima isn't going to reveal Irene's true last name we will never know. I think Erza wasn't even named by her mother but from one of the Village.
