It could be possible that you broke it. Over-use is a common reason why Action Replay breaks. I don't have it, so I wouldn't know. A lot of my friends do, though, and they say don't use it so often!

However, with the AR's for DS/DSi, then that is most likely NOT the case, provided you are taking basic care of it.
Personally, I've never seen a "codelist incomplete" error message, but perhaps one of these might help:
First: If you are using an ARDS and not an ARDSi, then it's firmware has to be updated to the most recent version to work with DSi-games (or any that are DSi-enhanced, such as Pokemon Black/White). Not all ARDS are capable of this, and generally it's really not worth your time if you want to be sure it'll work fine in the end. So, in that case, go get an ARDSi (works for DS/DSlite/DSi/DSiXL; NOT for 3DS), which costs about $20 (if it's higher, go somewhere else; also, you can order online).
Second: If you're using an ARDSi or a properly updated ARDS, then you may have too many lines of code active at once. As such, if you try to use, say most Pokedex-related codes, it is WAY too long for the AR and it will simply freeze (no damage, though; reset and it's fine).
However, you could have a code that is somehow itself written wrong. Use the AR Manager on your PC to make sure that it is completely done right.
Third: Make sure you are taking proper care and using the AR correctly. DO NOT EVER attempt to disconnect it while it is in operation; have the power fully shut down first. Also, DO NOT shut down the power while the AR device is saving data (while in the AR menus; once you have fully started the actual game running this is not a problem. Of course, don't turn off the power while saving data for the game either, as that is a potential risk to the game, despite the safeguards built in to it.).

You cannot obtain the Azure Flute in Pokemon Diamond, Pearl, and Platinum without the use of an Action Replay.
It's because the game you have inserted isn't in your ar's codelist if you have the program you can drag and drop the codelist from the "MY CODELISTS" section into the "NINTENDO DS" section. That should solve your problem.

With the appropriate codes, an Action Replay can get you any Pokemon that exist in your game and its predecessors (For instance, in Emerald you could get all the Pokemon in Emerald and the games before it)
