There is no "royalty" in The Sims 3. There are however nobles/kings/queens and such in the Sims Medieval, which is very much like Sims 3 but during Medieval times.

Yes. Currently, The Sims Medieval is available. Hopefully Sims 3 and other sims will be out soon.
Adventure packs in The Sims Medieval add new things such as quests, creatures and new Sims as well. In short, they are like expansion packs in The Sims 3.
