Bring a water talisman, a plain tiara then go to the water altar, and use the tiara on it, you will lose the water talisman, but you will obtain a water tiara.

The fire altar can be found directly outside of the duel arena in Al-karid. (Al-Karid is found by entering the gate east of Lumbridge)
You can make the at the water altar, buy them from a rune shop, or buy them from the Grand Exchange.
The altar locations are located by a "rune" icon on the world map. You can also find specific altar locations by Googling "Runescape Altar Map".
The water altar can be located at the Lumbridge Swamp.
That depends what you want to do with it. In the simplest case, to make water runes, the water tiara will simply allow you access to the water altar.

there is no steam altar. since steam runes are water and fire combined, you take your essence and a fire talisman to the water altar, or essence and a water talisman to the fire altar. you then attempt to create the steam runes (which have a 50% success rating, like smithing iron bars.)
