If it only does that for a certain disc, then the disc is probably damaged and needs to be replaced. If this happens for all discs, then there is a problem with your disc reader. If that is the case, contact Nintendo and see if they can fix your wii.

You cannot physically put a clutch disk in backwards unless you force it together. If you do this you will destroy the clutch disc and possibly other clutch components.

3 possibilities: either the disc is dirty, the disc drive is broken, or (you better hope this is not the case) the wii is broken. I'm not a wii specialist or anything so don't put all your trust with what I'm saying.
