James Earl Cash is executed for an unstated crime (likely murder), but shamed director Lionel Starkweather bribed prison officials to inject him a high dose of tranquillizer instead of lethal injection so James could star in a series of snuff films. When Cash awakes in a derelict part of Carcer City, he's told that there are gangs hired to hunt him down and kill him while Starkweather is filming and his only shot at freedom and second life is to fight his way out (kill or be killed).

It depends on the degree of conflict and action you would call violence. By popular opinion the game Manhunt is generally considered to be one of the most violent. Others would include any fighting game (Soul Calibur, Tekken, Street fighter), and most of the top selling games such as Grand Theft Auto and World of Warcraft involve violence.
You are a guy named Daniel Lamb. Daniel is in a hospital owned by The Pickman Project. Daniel wakes up and his celldoor is open. A guy named Leo Kaspar helps you to escape and get some details to get your life back. In the game you learn things about Daniels life and that Leo is your second personality.
