When u die thanks to a goblin, dragon, demon etc. U will keep 3 items. When u die with a skull above your head, which u can get by attacking someone in the wilderness, u will lose all your stuff you have with you.(You can keep 1 item thanks to activating the level 25 "Protect Item" prayer skill, which is used to keep 1 more item when u die, [Even when u die in the wilderness!] So by a normal NPC without anything above your head, you will keep 4 item's, [3+1], and in the wilderness u will still keep 1 item [0+1])Hope I helped ya!!
Going into they Abyssal Dungeon ( Zamorak-Mage Mini-Quest ), the same dungeon used for Runecrafting, you will be skulled on entry.Beware! If you are skulled, you will lost all items if you die, unless you have Prot. Item prayer on, you will save 1 item, that has most value in a general store.

When you die right now in Runescape you do get a grave.
When you die when your 'skulled' you lose all your items, unless you use the prayer 'Protect Item', and you'll save your most expensive item. You lose the skull, after you die, as well.
