I don't know which instance you're talking about but here are the two times that Gohan's arm got noticeably hurt. In the series Gohan is a teenager fighting Cell and his arm just gets broke in the last few little scruffs. In the movie "History of Trunks" the Androids 17 and 18 shot an Adult Gohan with a ki blast that took off his arm(In the alternate future where Trunks came from).
Shooting Star Dragon requires a 'Tuner Synchro' monster and a Stardust Dragon as components. Typically that will be Formula Synchron + Stardust Dragon, that's the easiest way. You can play around with alternate methods though, they are very inefficient. Armory Arm can be made into a Lv2 Tuner Synchro using Lightwave Tuning and Demotion. Or there are cards of different levels that can somehow copy Stardust Dragon's name. But it's probably best to stick with Formula Synchron for now.
