It makes uncommon dialogue very common.
This means that things your noble team, marines near by, covenant, and other talking AI will say different phrases. Things that would you would expect to hear are your teammates laughing, giggling, making jokes, catch phrases, etc when you turn the IWHBYD scull on. When off, they will speak normally and professionally. It does not effect difficulty or game play. If you mute your TV, you wouldn't know the difference from the scull being on or off.

You're probably jumping through the rings in the correct order, but if you are touching the rings the skull will not appear. It also has to be on Normal or harder difficulty.
