Some are limited or semi-limited, meaning only one or two per deck. But if the card is not forbidden or Limited in some way, you can use three in your deck.

Depending on how they see whether a card should be banned is entirely up to them. Also if a new card comes out that when combined with an already existing limited card causes games to end really quickly then the limited card will become forbidden and then when the sort of deck that the combo was used it dies out then the card that became banned will become limited again. (and in some cases the cycle will repeat itself).Tournaments are the reason. If there is a combo that is too good, then they will either limit the cards in it, or forbid one or more completely. But, if they do forbid a card, they often times take other cards off the forbidden list. Such as Monster Reborn being banned, and Call of the Haunted being removed from the banned list and only limited.
It's like they don't mind players being able to special summon from the graveyard like that, just as long as there aren't too many effects that can do it easily. When we get Monster Reborn back, we've usually lose Premature Burial or Call of the Haunted at the same time.
