In my opinion, defence for higher levels is not that hard, but takes time. Attack and strength are good to raise. I recommend training all of them.
to go up in combat level you need to go up levels in strength, attack, defence, hitpoints, prayer and summoning.NOTE: Magic ,range ,and and slayer do put up your combat level but it takes much higher lvls for them to affect it, no other skills do affect combat lvl.The highest combat level you can get is 138, this is when you have level 99 in all strength, attack, defence, hit points, prayer and summoning
When you attack, make sure your attack style is 'block'. This means when you attack something, you gain experience in defence only.
The scimitar is a good all round weapon. For your level a mithril one will probably suffice.
It depends what your strengh is, if your strengh is higher you should get a warhammer or battleaxe. Assuming you had 99 Hitpoints, and only had 1 Prayer, Summoning, Magic, and Ranged, you would have a combat level of 96.
Your combat level is also dependant on your hitpoints, prayer, range, magic and summoning level. Assuming you had 70 attack, strength and defence, with 10 hitpoints (the lowest level you can have) and the rest of the skills mentioned above at level 1, you would have a combat level of 65. If however you had 70 hitpoints as well, you would be a level 80 combat.

To Wear/Wield void, you will need level 42 attack, strength, defence, hitpoints, range and magic and level 22 prayer.
yes by getting up your strength def attack magic range and hitpoints .Also prayer, and summoning.
Only slightly, you will also need more strength and attack than defence to lay harder hits.
