If you're talking about a minecraft world, there is no limit. The world keeps randomly generating. If you're talking about the map item, it changes. As of 1.4 you can expand a map to make it show more blocks.
It depends where you stand. Mountains, seas and regular plains will all have different block levels. On your Minecraft map you will see x, y and z coordinates. Y is the number of blocks you are above the bottom of the earth. Y:12 is the 12th layer.
You can only find diamonds in the bottom 16 layers of the map. You can find which layer you're in by hitting F3 (or fn+F3 on a Mac); look for your "y" coordinate.
Yes - and the location is different in every map. It will generate between 640 - 1152 blocks (in both the x and z) and the numbers can be negative as well, for example, x: -1100 z: 800

Some might, but an adventure map is a MAP, not a mod. Just search for "adventure map" on Google or the Minecraft Forum.
Use a mod called 'world edit' or get a program called MCedit, with it you can copy and paste blocks that you select.
