In Creative Mode, you must have the End Portal Frames in your hand. Stand on a flat piece of land, or 5 x 5, and get in the middle of where you'll be placing the portal. place 3 portal frames 1 blocks away from you adjacently and continue this until you can't anymore. Then put eyes of ender in them facing the inside of the portal.
