To get the flaming helmet on Halo Reach you had to pre-order the legendary pack. The "legendary" version of Halo: Reach includes the game and an assortment of items designed for hardcore fans of the sci-fi series. The main selling point for this edition is the hand-painted statue of Noble Team, created by McFarlane Toys. Measuring at ten inches in height and weighing over ten pounds, the individually numbered collectible features five Noble Team members positioned against a rock. Also exclusive to the legendary edition are UNSC themed packaging and a Spartan armor effect for use in multiplayer games. Everything from the limited edition is included as well, consisting of an in-game Elite armor set for multiplayer competition, an embroidered Spartan II patch, an artifact bag containing Dr. Catherine Halsey's journal, and a replica of Dr. Halsey's ONI security badge. All of the limited edition's contents are housed within a custom-created box that recreates the look of an ONI data module

Codes for the Flaming Helmet were included with the Legendary Edition (MSRP $150.00 USD) of Halo: Reach. The only way to obtain them is to buy a Legendary Edition version of Halo: Reach, have a friend give you an unused code, or mod it (and get banned).
The "Flaming Head" is not actually a Head Piece. It is a "Chest Piece." It resembles the EOD Variant with a flame on the Collar. Because of this, ANY "Head Piece" can be "Flaming."

Whoever told you that there is an app for the iPad that allows you to receive the blue flaming head attachment knows that you are extremely gullible, probably not all there in the head, and easy to lie to. There is no app that allows you to receive this attachment, it is exclusively accessible to Bungie employees.
