yes it will come back if you defeat the E4 and Lance. It will keep being that until you catch them. Same rules apply to other legendarys

you cant groudon (like other legendayrs) are a one time capture Pokemon, you get one shot and that's it you should save your game just before you fight them to insure this doesn't happen

Unfortunately, no you can not access Kyurem's Lair again, unless you didn't catch it or accidentally made it faint

You can always fight the Elite Four again. That allows you to find Mewtwo again. It worked for me in Pokemon X. It is best to save before you make it faint. Do so BEFORE you enter their caves! SAVE BEFORE BATTLING LEGENDARIES.

Where is azelf if he is not in his cave?
You Have To Complete The Spear Pillar Scene And It Should Be There. If Not You May Have made it faint... You cannot get azelf unless you catch or defeat Dialga or Palkia at spear pillar.
