No, you will not loose any of the items you came in with at Castle Wars even if you are skulled. Castle Wars is a safe mini-game and there is no possible way to loose your items (except runes and ranged ammo) unless you drop them on purpose. Being skulled means nothing inside mini-games like Soul Wars and Castle Wars...people go there often to loose their skull.

After time the skull will disappear if you don't attack anymore people during that time. When you are skulled and die you will lose all items unless you're using prayer to keep one extra. For this reason you must be careful when skulled, but it might be a risk worth taking depending on the situation (it is much less risky if you have teleport runes or are near a safe area). Runtkirk3: As of the free trade and wild update there are no longer pvp or bounty worlds. The way to get skull is to go into the wild and attack another player befor they attack you. But that isn't the only way.. you can also go into the abyss to get skulled.
