It tastes a bit...off. But since it's not really milk but corn syrup with sodium and calcium caseinates ....I expect it to taste just as off until that moment comes when the caseinates holding this 'milk' substance together will collapse and it will come out of solution and just...be nasty, however even then, I doubt it would go 'bad'. Just more unpleasant.
Otherwise, with the 2006-2008 ENSURE I have, I just shake them rather vigorously for about one minute. This makes it taste less bitter. Why? Let that be the next question then...

there is no expiration date however it may go flat. but, if it was stored correctly it shouldn't ever go bad.

All you can say for sure, is the manufacturer will not guarantee the drug is 100% effective after the expiration date.

There's no way to tell you that. Too many variables are involved. Use your senses to tell if it is spoiled. Check for off-odors, sliminess, mold and discoloration.
