Lewis Levy is, at the very least, the character name of a philosopher/theorist that Woody Allen and Mia Farrow watch interview footage of in Mr. Allen's film Crimes And Misdemeanors. I just recently rewatched it, and was struck by the footage shown of this man, but could find no mention of him in Woody's credits. The man's real name? Real footage?

Nikko was the name of the king of the flying monkeys listed in the credits of the 1939 movie. Neither the book nor the plot of the movie ever mention the name Nikko.

I wish I had the answer, I've owned the vinyl album since 1977 and to this day I still don't know who played the guitar on Chameleon or why there is no mention of a guitarist in the credits. I thought maybe it was a clavinet made to sound like a guitar.
Unfortunately, I don't think she wanted us to know as it does not mention. She went to the Italia Conti academy of theatre arts if you wanted to know. You could try checking the credits at the end in case they give her last name.
